I’m pretty sure it has something to do with him becoming more snake like because in the books he drinks Nagini’s milk
Voldemort's body is destroyed as we know how and in 4th part Wormtail/Peter Pattegrew recreated his body but before that he was not in good condition thats why he has to drink nagini's milk
When he created horcruxes, his appearance changed because of the Horcrux Ritual. His face become more snakelike, atleast he got some nostrils for breathing

Maybe because when he became Moldy Voldy, he gave up parts of his soul. Which, in turn, broke up his appearance, which also explains why he's super pale. Can anyone confirm or no?
No he were in normal condition even he has gave uped his soul parts

Voldemort used to take over the bodyies of Snakes according to the books which is why he doesn't have a nose since he is used to not haveing one and so all of his bodyies after his original body so it is a thing of habbit for him
