In the 3rd movie of the HP series, PoA, Sirius talks to Harry in front of his family tree and we can see his name and also Bellatrix's name too.
I thought Bellatrix belonged to the Lestrange House...

Yes. Sirius is Bellatrix’s first cousin. But after Bellatrix married Rudolphus Lestrange she changed her name.
Yes they are related but because bellatrix got married she changed her last name and it was in the 5th book not the 3rd
Well, in the books it says that all pure blood wizards and witches are related in some way.
