In Philosophers stone, when Harry has become a seeker, he is with Ron, Fred and George. We're the twins joking when they said "no one's died in years" or were they being legitimate.
I don’t think anyone had recently died playing Hogwarts Quidditch. Its a dangerous enough game that there probably had been deaths at some point though. So I’d say they were being honest. Although they obviously brought it up as a joke.
Nearly every sport in history occasionally has an accidental death. It’s a tragedy but also a reminder to wear proper protection and to play by the rules. Deaths in sports are very rare though, so as long as you aren’t being wreckless you should be ok.
Certainty not final canon, but Willa Weholt from Magic Awakened comes to mind, who died trying to catch the snitch in a Quidditch match quite some time ago.
