It's not a difficult concept to grasp. They're standing in the hospital wing and use the timeturner therefore being transported to an earlier time in an empty room. Then they go back to where they were supposed to be at the time. Everything takes place in the same order and when future Harry and Hermione return the past ones have gone. What are you not understanding?
