Fairy Cakes to Fairies (incantation unknown) was a transforming spell that turned Fairy Cakes into actual fairies.[1]

During the 1989–1990 school year at Hogwarts School of Witchcraft and Wizardry, Professor Minerva McGonagall taught this spell to her sixth-year students in Transfiguration class as part of their N.E.W.T.-level curriculum.[1]
