The latter had been sliced apart by Tom Riddle's snake Nagini. He was quickly dying, and urged Harry to take the memories that flew from him as his life drained away. Hermione, her hands shaking slightly, conjured this flask to capture the memories in.
Use and post-use
Harry took the flask with trembling hands and carefully retrieved his late Professor's memories at the latter's urging. When the last of Snape's life had left him, Harry returned to the castle where he viewed Snape's memories in Albus Dumbledore's Pensieve.
It is unknown what happened to the flask; it presumably ceased to exist as all conjured objects tend to over time.
