This important document was a form issued by St. David's Hospital. It was, at one point, accidentally shredded by Nurse Michael Babatola, which made him nervous enough to eat a whole box of chocolates that had been sent there by Sara Farnham to Bilius Finbok.[1]
While looking into Finbok's disappearance, Ministry of Magic investigators Mathilda Grimblehawk and her partner, offered to "handle" the issue to show appreciation for Nurse Babatola's help in the investigation — after having her partner magically Mend the paper shreds while the nurse was looking the other way, Mathilda handed the document back to an amazed Babatola, saying her partner "does a lot of jigsaw puzzles". Afterwards, Babatola agreed to go and apologise to Sara Farnham in person.[1]
